Mission StatementWe are committed to building relationships that offer personalized care and oral health education. We strive to provide quality dentistry that families can be confident in and trust.

Office Hours
Our office will be open Monday through Thursday from 8 a.m. to 5 p.m.
The doctors reserve Fridays to treat patients in the hospital in Evansville Indiana.
The office will be closed on weekends and major holidays. At times, the
office will be closed when the doctors and staff are attending continuing
education programs.
Emergency and After Hours
We will make every effort to see emergency patients as soon as possible.
Calling as early in the day as possible for the emergency will allow us
more time to fit your child into our schedule. A 24-hour answering service
is available by simply dialing 853-2977. You will be given information
as to how to contact Dr. Miller or Dr. Cook. If there is no emergency,
you may leave a message and our staff will return your call during routine
office hours.
Appointments
Your child will be seen by appointment only, so please call in advance
so that we may reserve a special time for you. We make every effort to
be on time for our patients and ask that you extend the same courtesy
to us. If you arrive late for your specially scheduled appointment, in
order that we don’t make patients wait who are on time, we reserve
the right to reschedule your appointment for another visit. We request
that the person bringing your child to the office stay at the office until
the appointment is finished. Often important questions are asked during
treatment.
Appointment Commitment
Please remember these times are reserved exclusively for your child. Any
changes in these appointments affect many people.
Telephone Calls
For the doctor to leave a patient during treatment to answer phone calls
would be unfair to the patient. The office staff has been well-trained
to answer most questions. If they are unable to answer your questions,
arrangements will be made to have the doctor return your call.
No Smoking Please
Since exposure to tobacco smoke is detrimental to your health and others,
we ask that you refrain from smoking in this office. We would like to
thank you in advance for your consideration.
Recall
The most important part of dental health is prevention. We can’t
emphasize enough the benefit of regular preventative dental exams at least two
times per year. However, the responsibility of returning for these visits
is ultimately up to you. To help you reserve an appointment, we will schedule
your recall or cleaning visit six months in advance. We will remind you by mail approximately two weeks prior to
your visit. When returning for a recall visit, please advise the office
staff of any changes in your address, telephone number, marital status,
employment, insurance coverage and health status of your child.
