Fees and Payments
Unless otherwise previously arranged, payment is expected as services are performed. When treatment is required, you will be given an estimate and will be asked to make definite financial arrangements with a member of our business office staff. This will lessen confusion and provide better understanding. Payment is considered deliniquent after 30 days regardless of Insurance coverage.

Insurance
Our computer system greatly simplifies preparation of insurance claims for you. We use the American Dental Association universal claim form and also file claims electronically. Simply bring your specific insurance form with your portion completed and signed by the appropriate parent. One form is needed per child. We will then keep this form for additional claims. If you change insurance companies, please inform the business office staff and bring a new claim form. Appropriate insurance cards will also be sufficient. If there are two insurance companies participating in the claim, we will file the primary insurance company only. This gives you the necessary information to personally file with your second participating company. We will do our best to help you receive the maximum benefits available under your policy. However, please remember that the relationship is between you, the insured, and your insurance company. Your financial relationship for treatment is between you and our office. As a courtesy to you, our office files the insurance claims with your company in order that you obtain maximum coverage from your insurance.
